I can't manage to insert a YouTube video

This is a great tool! But I now tried to insert a YouTube video - and it does not work. I get "500 Server Error".
What am I doing wrong?

Good, but I'd like to see more fields in the report

I'd like to see customer zip code and city in the report - maybe coming in an update?
